Home Based Services
Available to customer/owners in the Anchorage area, a variety of home-based services are provided, including acute care upon discharge from the hospital, chronic care for disease management and medication management and palliative care. Case management is available for infants, children and adults of all ages.
- Skilled Nursing services include medication management, wound care, ostomy care, pain management, infusion therapy, case management and education.
- Home Health Aides provide assistance with daily activities, range-of-movement and home safety.
- In-home physical therapy is subject to availability of therapist.
This department is staffed by registered nurses, licensed practical nurses, and certified nurse aides. Social worker services are also available.
